This release form must be completed by the exhibitor. You may print it out, complete it and bring it to the library or a copy may be obtained at the library.

I, the undersigned, hereby lend the following works of art or other material to the Bethel Public Library for exhibit purposes only. In consideration of the privilege of exhibiting them in the library, I hereby release said library from responsibility for loss, damage, or destruction while they are being displayed at the Library.

Further, I hereby agree to indemnify and save harmless the Town of Bethel, its agents and employees from all loss and/or expense (including all costs and attorney’s fees) arising out of or resulting from the exhibit by reason of liability imposed by law upon the Town of Bethel, its agents and employees from damages because of bodily injury, including death, at any time resulting there from, sustained by any person or persons or on account of damage to the property, including the loss of use thereof, whether such injuries or damages are caused in whole or in part by the negligence of the exhibitor, its agents, servants, patrons, and/or employees.
